The UK general practice has reached crisis point. The impact of the Covid-19 pandemic has placed a strain on an already crumbling primary care service, leaving both patients and NHS staff struggling.
Seventy-six years after the NHS was created, Dr Ellen Welch and Dr Nighat Arif lift the curtain on general practice, both reflecting on the history of the profession and how it’s changed, and looking ahead to what general practice might look like.
With accounts from GPs across the country, those on the frontline try to answer the question: how did we get here? Is it better overseas? And what can be done to make things better for us all in the future?

Dr. Ellen Welch, a GP and mother, previously co-chaired the Doctors’ Association UK. All proceeds from her latest book going to support DAUK. With titles like How the NHS Coped with Covid-19, she sheds light on healthcare challenges. A prolific writer, her contributions extend to publications like The Guardian and BMJ. Based in Cumbria, she offers insights into the NHS’s journey.
Nighat Arif
Dr Nighat is a GP specialising in women’s health, family planning and menopause care with over 16 years of experience in the NHS and she runs her own private practice. She is the resident Doctor on BBC Breakfast and ITV’s This Morning. She is a health content creator working with YouTube Health and the WHO Fides programme. She is the Ambassador for Wellbeing of Women and the recipient of the 2023 Prime Minister’s ‘Point of Light’ Award in recognition of her work of raising awareness of Women’s Health issues in the UK. She is the author of The Knowledge: Your guide to female health from Menstruation to the Menopause.
